HOW TO SEND FILES?
If you only have one photo or scan of your medical report (.pdf or .jpg format) & ndash; just add it to the carefully filled form. IMPORTANT: The document must be readable.
If multiple documents: Copy photos or scanned copies to a separate folder named after the patient. Archive the folder to .rar, .zip, or .7z and add this archive to the completed form.
MRI / CT scan disk: Copy all files from the disk to one folder named after the patient. Archive this folder to .rar, .zip, or .7z and add this archive to the online consultation form.
